You are eligible for the scheme if you:
- Aged between 10 and 18 years old
- Overweight (according to the NHS child centile scale) or will benefit from exercise for your mental wellbeing
- East Riding resident
- Are committed to lifestyle change

To join this scheme, you need to be referred by one of the following:
- East Riding GPs
- School Nurse
- Parent or Guardian
- Carer
- Dietician
- Physiotherapist
To get a written referral, book an appointment with one of the above practitioners and ask about the HealthiER scheme at East Riding Leisure.

Parents and guardians are also able to refer their child for the Young Live Well programme:
If you are eligible, your health professional, or parent/guardian should send a written referral by email to the HealthiER team.
If you need to call the HealthiER team, their office is open Monday to Friday, 8am to 4.30pm.
Young Live Well is free to eligible participants.
Additional optional Young Live Well Group health class sessions are available; you can check availability and book these online.
